We'd love to see you, but with MyChart we don't have to. Free and available to all Luminis Health Doctors Community Medical Center (LHDCMC) patients, this secure and convenient tool gives you round-the-clock access to your medical information — and to your doctors — no matter where your day takes you. Available through any smart device or computer, MyChart offers communication on the go anywhere you are.


Manage Your Care

MyChart provides a fast and easy path to your personal health records. Use it to view:

Appointments

If you need to see your primary care provider, MyChart makes scheduling an appointment fast and easy. Whether you’re at your desk or on-the-go, it’s as simple as a few clicks. Just find an available appointment time that works for you and reserve it.

Participating providers also offer video visits through MyChart depending on your health concern. Video visits are only available through our Luminis Health app, available for download on both Apple and Android devices. You must already be an established patient in the MyChart system to be eligible for video visits.

There may also be times when it’s tough to remember everything discussed at a past appointment. Use the ‘Visits’ tab to refresh your memory by reviewing the visit summary, treatment recommendations, and other details of any past appointments.

Current Health Status

Your providers will document any current health concerns and treatments into your MyChart record. You can log in from any computer, phone, or tablet to review your provider’s notes, verify your treatment recommendations, and learn more about your diagnosis.

Health Trends

Since MyChart captures your vital signs as your doctors take them — important health indicators like your blood pressure, weight, and pulse — you can monitor your progress or any potential health concerns over time.

Immunization Records

See or print your vaccination history, and get recommendations for future immunizations based on your age, gender and condition.

Medications

Get a convenient list of your current medications and dosages, and access to a library of health information so you can learn more about them.

Recent Test Results

We will automatically load most test results, including all recent imaging, lab, and pathology results into your MyChart record. However, due to the sensitivity of some cases, your doctor may withhold your results from MyChart and ask to speak with you in person first.

No Waiting Room Necessary

Managing your own care has never been easier. MyChart offers a variety of ways for you to communicate with your doctors and care team through your computer, smartphone or tablet. MyChart also helps you stay on top of important health information with reminders of upcoming appointments, prescription refills and more.

Message Your Care Team

Get answers to your medical questions from the comfort of your own home. Leave a message for your doctor or care team and hear back within two business days.

Pay Your Bill Online

Medical bills can be a lot to manage, especially if you’re dealing with a serious or chronic condition. MyChart gathers your bills from all of your LHDCMC providers in one convenient place and gives you the functionality to message the billing office or make payments online. You can send one-time payments or set up automatic ones — the choice is yours.

Refill a Prescription

Stop all the phone calls and cut down on your wait time by requesting your prescription refills through MyChart. You’ll be glad you did.

Schedule a Video Visit

If you need to talk with a doctor about a minor health issue, video visits are a fast and convenient way to get care — especially when you don’t feel like leaving the house.

To schedule a video visit, first select the reason: a follow-up or a sick visit. Then you will complete a short questionnaire describing your symptoms. If a video visit is appropriate and available for your medical need, you will be offered a video visit and prompted to select a convenient time. Your doctor will meet you by video chat at the designated appointment time to discuss a diagnosis and treatment plan. If medication is prescribed, the prescription order will be sent directly to your preferred pharmacy.

Family Access

If you’re a parent or caregiver, adding your loved one to your MyChart account will allow you to view their information and manage their health care online. There are three versions of “proxy access” available: one for children under age 13, one for teens aged 13-17, and another for legal guardianships. To request access, complete a Proxy Consent form at your provider’s office or download the appropriate form below. Be sure to include any supporting documentation and mail everything to the address listed.

Enroll

It's easy to enroll in MyChart. Simply request a MyChart activation code online or during an office visit with any participating practice. Your activation code enables you to log in and create your own user ID and password. If you are not already enrolled after being discharged from the hospital, emergency department, or an outpatient office, you can request an account by calling MyChart Support at 833-254-3600.
