If you have been treated at Doctors Community Hospital, you may have questions regarding your health record, want to view your health record, or need a copy of your health record.
For questions or to schedule an appointment to view your health record, contact the Health Information Services (Medical Records) Correspondence Department at 301-552-8073.
If you wish to obtain a copy of part or all of your health record, please click on the Authorization to Disclose a Health Record* to print a copy of our Authorization form. We cannot release health records over the Internet; however, with this completed form and photo identification, you can obtain a copy of your health record. There may be a fee for copying services. Please mail the completed form to:
