BENEFIT CHOICES
DCH is committed to providing you with a comprehensive variety of benefits. These benefits are a significant and important part of your total compensation package. They provide valuable protection for you and your family.
MEDICAL
Prevention is the best medicine; CareFirst BCBS offers a wide range of services to help employees and their families lead healthy, productive lives. Three plan choices through CareFirst BCBS: BlueChoice HMO, Point of Service (POS), or Blue Preferred PPO.
DENTAL
Dental insurance is available to help manage the cost of dental care through Delta Dental. Two PPO plan choices Core Plan ($1,000 calendar year maximum) or Buy-up Plan ($2000 calendar year maximum)
VISION
Vision benefits include an annual eye exam, eye glasses or contact lenses through Vision Service Plan (VSP).
LIFE INSURANCE/AD&D
In order to provide a full range of protection, life insurance coverage is provided at no cost to Full-time employees at 1x their annual salary. Part-time employees may elect life insurance for a nominal cost.
SUPPLEMENTAL LIFE INSURANCE
To protect your family’s financial resources this optional benefit is available with choices from 1x to 5x annual salary.
DEPENDENT LIFE INSURANCE
Coverage levels available to choose for your:
Spouse: $4,000, $5,000, $10,000 or $25,000
Child(ren): $2,000, $5,000 or $10,000
WHOLE LIFE INSURANCE
Boston Mutual offers this benefit to your entire family. The policies are individually owned and build cash value. Coverage is guaranteed during initial eligibility period and no physical exams are required. The cost will never increase.
SHORT TERM DISABILITY (STD)
This benefit is designed to provide you with a source of income in the event that you become disabled and are unable to work. Boston Mutual provides coverage up to 60% of your salary and benefits begin on the 8th or 15th day of an injury or sickness and paid up to 6 months.
LONG TERM DISABILITY (LTD)
This long term “paycheck protection insurance” is designed to provide supplemental disability coverage. Boston Mutual offers coverage up to 60% of your salary. Benefits begin after 6 months of continuous disability and paid up to 5 years.
EMPLOYEE ASSISTANCE PROGRAM (EAP)
Personal, family or work-related challenges can arise at any time. That’s why it is important to have easy access to professional, caring counselors 24 hours every day. This benefit is free to every DCH employee and family members.
PAID TIME OFF (PTO)
This employee benefit combines holiday, vacation and sick leave programs into one plan to provide both employees and DCH with a flexible method of scheduling time off with pay. PTO accrues based on length of service:

AFLAC (VOLUNTARY BENEFITS)
Accident plan covers out of pocket expenses and lost wages;
Sickness plan covers physician visits, initial hospitalization, hospital confinement, major diagnostic exams and surgical procedures that result from an illness;
Cancer plan is designed to relieve the financial strain of a catastrophic illness. Benefits include lump sum first occurrence, hospital confinement, radiation and chemotherapy treatment;
Specified Health Event plan is designed to offset expenses due to heart attack, stroke, coma, and 3rd degree burns;
Intensive Care plan covers intensive care unit confinements due to an illness or injury.
LEGAL PLAN
The option to elect legal services from Legal Resources.
TUITION REIMBURSEMENT
DCH promotes the development of its employees through education. Tuition cost will be reimbursed up to $3,000 for degreed courses, or up to $1,500 for non-degreed courses per calendar year. Benefits are prorated for Part-time benefit employees.
CATASTROPHIC ILLNESS/INJURY LEAVE BANK (CILB)
This voluntary benefit allows members up to a 12-week PTO grant upon diagnosis of a catastrophic illness/injury. Members contribute an annual specified amount of PTO to participate.
RETIREMENT
Cash Balance Pension Plan: DCH provides an annual contribution equal to 3% (first 10 years of service) then 4% (10+ years of services) of your total compensation for that calendar year. Three year vesting service required to receive benefits with various payment options when you retire or terminate.
403(b): Employees can contribute pre-tax dollars into investment options provided by AIG Retirement. DCH provides a match to benefit eligible employees up to 2% of your contributions.
MODIFIED EMPLOYEMENT PLAN (MEP)
This benefit allows employees to waive ALL benefits in lieu of a higher salary.
RN & BSN SCHOLARSHIP and JOB PLACEMENT PROGRAM
The scholarship program provides financial assistance for your nursing education up to $4,000 per year for nursing students enrolled in an associate’s degree program (ADN) and up to $8,000 per year for students enrolled in a bachelor’s degree program (BSN). For each year of scholarship received, you will owe one year of full-time employment at DCH.
